Anyone can get laryngeal cancer, but you might be more at risk if you:

* are over the age of 40
* are a man, especially if you are Black
* have a brother, sister, parent or child who has had laryngeal cancer
* have certain conditions, such as acid reflux or gastro-oesophageal reflux disease, or have had radiotherapy treatment to your neck
Many laryngeal cancers are linked to lifestyle. For example, you’re more likely to get it if you smoke or drink too much alcohol.

Don’t ignore the symptoms

Systemic therapy remains the cornerstone of treatment for recurrent and metastatic (R/M) head and neck squamous cell cancers (HNSCC). However, there is a dearth of effective treatments beyond platinum combinations, anti-programmed death-1 (PD-1) agents and the epidermal growth factor receptor (EGFR)-targeting monoclonal antibody cetuximab. Recent years have seen several exciting new agents being tested in clinical trials. These are designed to target alternate oncogenic signaling pathways and have novel mechanistic compositions, including bi-specific antibodies and antibody-drug conjugates. 

A new study published in Frontiers delves into the clinical limitations of currently approved systemic therapies, explores novel agents in development, and highlights ongoing clinical trials using targeted therapies for this disease.

In head and neck cancer (HNC), various cortisol indices are known to correlate with adverse phycological and biologic (eg, inflammatory) outcomes, but links to mortality rate has not been investigated. A research group led by Elizabeth Cash conducted a study to hypothesize that the prognostic value of diurnal cortisol aberrations will hold in HNC.

The results, published in Frontiers in Oncology, builds on prior work that led the research team to predict that flattened or elevated diurnal cortisol profiles will be associated with elevations of serum inflammatory and tumor-promoting cytokines in this population and that these immune markers would predict poor progression-free survival.

A pilot sample of 40 patients with HNC were recruited; most presented with late-stage oral/oropharyngeal cancer, were older than 50 years, were men, and subsequently received combined-modality (surgery and/or radiotherapy with or without chemotherapy) treatment with curative intent.

Saliva was collected twice daily for six days to assess diurnal slope, mean, waking, and evening cortisol levels.

Bivariate correlations, linear regressions, and Cox proportional hazards models were used to test the hypothesis that disrupted diurnal cortisol rhythms would be associated with poorer two-year progression-free survival.

Elevations of evening cortisol and diurnal mean levels were each associated with shorter progression-free survival.

According to the research group, the results suggest that cortisol secretion is both dysregulated and elevated among patients who subsequently experienced accelerated HNC disease progression. These data from the 40 patients with HNC mirror relationships between cortisol levels and survival identified among patients with various other tumor types.

Cases of mouth cancer have reached a record high in England and Scotland. Last year, there were more than 10,000 new cases.

Mouth cancer has increased by 38% in the past decade and by 133% compared with 20 years ago. Last year, more than 3,500 people in the UK lost their lives to mouth cancer.

Several factors are probably behind this increase in mouth cancer cases. Changing lifestyle habits, particularly those related to known risk factors, play a significant role.

Tobacco use, including smoking and chewing tobacco, remains a primary cause, with around two-thirds of cases directly linked to smoking. Excessive alcohol consumption is another major contributor, responsible for about one-third of all mouth cancers.

The combined effects of heavy drinking and smoking can increase the risk by a staggering 30 times.

Additionally, the human papillomavirus (HPV) has emerged as an increasingly important risk factor, particularly for oropharyngeal cancers, a type of mouth cancer affecting the throat. Oral HPV is thought to mainly spread through oral sex.

How to spot it

Mouth cancer can be effectively treated if caught early enough. Common symptomsinclude persistent mouth ulcers that don’t healwithin three weeks, unexplained lumps in the mouth or neck area, white or red patches in the mouth or throat, and difficulty swallowing or persistent hoarseness.

Regular dental check-ups and self-examinations can aid in early detection.
